At CPH Disability Support, we customize each participant’s program to meet their unique needs and family goals. Our dedicated team works closely with families to ensure personalized and effective support for a fulfilling and independent life. Based on the participant themselves and the best way to meet their care, this may include the following and more:
- Assistance with daily living: dressing, grooming, personal hygiene, toileting, continence support, eating, drinking
- Medication management and administration
- Mobility and transfer support: walking, use of mobility aids, bed-to-chair transfers
- Health and wellbeing monitoring: vital signs, exercise assistance, health appointments
